Lessons from Major Radiation Accidents

Lessons have been learned from investigations into a relatively large number of accidents that have occurred in three major practices, namely: industrial radiography, gamma irradiators and electron accelerators used in industry and research, and radiotherapy. This paper provides an overview of the lessons learned and specifically considers the human actions and omissions that have resulted in an accident.
